四川在线记者 何嘉琦 魏冯 肝内胆管癌恶性程度高、进展快，不少患者经多次化疗、介入治疗后，肿瘤仍会侵犯肝脏关键血管，错失常规根治手术机会。针对这一临床困境，四川省人民医院大外科主任、肝胆胰外科主任张宇教授团队创新应用“在体冷灌注”技术，突破传统手术限制，为晚期复杂肝肿瘤患者带来根治新希望。什么是“在体冷灌注”技术？四川在线记者邀请到张宇主任，为我们答疑。 传统复杂肝肿瘤手术常面临两大难点：肿瘤侵犯血管时要么无法切除，要么只有极少数高水平团队才能进行手术，但依然面临出血、血栓和感染风险，手术风险高、预后差。为此，张宇教授团队历经四年攻坚，探索出无需离体、无需静脉转流的“在体冷灌注”创新技术，简化复杂手术流程，降低手术风险。目前，团队已采用“在体冷灌注”技术为130余例复杂肝脏肿瘤患者完成切除手术。 张宇主任介绍，近日该团队凭借这项新技术，成功救治一名晚期肝内胆管癌患者。患者术后恢复良好，次日转回普通病房，五天即可正常进食，康复进展顺利。“相比传统术式，具有创伤小、风险低、易推广的优势，还可融合多种肿瘤治疗手段，大幅提升难治性肝肿瘤的根治率，拓宽了肝胆疑难肿瘤的手术治疗边界。” 张宇主任表示，依托上百例临床实战经验，团队创新性地提出了针对该技术的“五型分类法”，根据肿瘤血管侵犯特征精准分型，明确各类病症的标准化手术方案与灌注路径，让疑难肝外科手术从“经验行医”迈入标准化、规范化的精准医疗时代。 目前，这项技术已拓展应用于胆道、胰腺等多部位复杂肿瘤手术。未来，团队将持续优化技术标准、推广创新成果，让更多肝胆胰肿瘤疑难重症患者重获生机。
